This form is for sponsoring Lions clubs to report their Leo Districts and Multiple Districts Officers and Chairpersons. This should be done each fiscal year, either through MyLCI, but if they do not have access to MyLCI, then they use this form.

Leo of the Year is a prestigous award presented to Leos that have demonstrated outstanding leadership and positively impacted their Leo clubs and community. Each multiple district may nominate one Leo annually. This is the nomination form.
